Description (Service) | First Link to information and support for individuals and families who are living with dementia, mild cognitive impairment or are at the greatest risk of developing dementia.

Application | Call 519-428-7771 if you're concerned about memory loss or dementia diagnosis - no referral is needed
Health Care Professionals can refer patients/clients to programs/services or request the support of a psychogeriatric resource consultant or behaviour supports ontario worker. Physicians may request cognitive testing for patients who reside in Haldimand Norfolk. Referral forms are on the website: www.alzda.ca, under the Health Care Professionals tab |
