Description and Service Details
Description (Service) Halton residents are responsible for arranging their own transportation to and from their vaccination appointments 
* if you are able to, please drive yourself or make arrangements for a ride from a family member or a support person 
* if you will be traveling to a clinic with someone from outside of your household, please review the COVID-19 Public Health Guidelines: Car Travel for tips on how to travel safely 
If transportation is a barrier to getting to your appointment, please choose one of the options below: 
Option 1: Request a no-charge ride through one of the local taxi services 
* to have the cost of the taxi service covered you must provide the taxi service with your vaccination appointment Confirmation Number 
* please book transportation at least 48 hours in advance of your clinic appointment 
* Burlington: Burlington Taxi - 905-333-3333

* Oakville: Oakville United Taxi - 905-338-0044 
Option 2: Request a ride through the Ontario Community Support Association (OCSA) 
* the Ontario government has partnered with the OCSA to help people with disabilities, and seniors with mobility issues, safely get to and from COVID-19 vaccination clinics 
* to book a ride through the OCSA you need to: have a vaccine appointment booked, be a person with a disability (within the meaning of the Accessibility for Ontarians with Disabilities Act, 2005), and have limited or no access to safe transportation or face barriers to getting to or from a COVID-19 vaccine appointment 
* one caregiver or support person can go with you, to and from the COVID-19 vaccine clinic 
* visit website to book 
Option 3: Arrange for a Paramedic to administer your vaccine at your home, through your primary health care provider 
* if you have mobility challenges or medical reasons that prevent you from attending a vaccination clinic, you may be able to have a Paramedic vaccinate you in your home 
* this can be arranged through your primary health care provider completing a ‘Physician Request Form for COVID-19 Vaccination of Homebound Patients’ and to submit it to 
* for further details about this process, have your primary health care provider refer to the April 23, 2021 e-fax to physicians
