• Remote Care Management (RCM) Program: partnership between Halton Healthcare, Connected Care Halton Ontario Health Team, Ontario Health atHome and Halton Community Paramedicine * patients on this program have their vital signs reviewed by a nurse remotely * patients with acute viral resprtatory illnesses, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), palliative with a prognosis of 2 to 12 months can enroll
