Description and Service Details
Description (Service) An initiative of community safety and well-being in Halton * pilot project focusing on adults 55+ who are at risk or currently experiencing social isolation * partnership with organizations that support older adults in Halton Region in various capacities

Services include:
  • one-on-one assistance with a Connection Specialist who is specially trained to support adults 55 and older with accessing information and resources in their community to increase their support network
  • Connection Specialist will work collaboratively (either over the phone or in person) with the community member, facilitating links and coordinating services that will meet the individual’s needs and reduce isolation

Signs that someone may be socially isolated include:
  • physical health changes such as bruising, weight loss and weakness which can be a result of declining health or possibly a sign of elder abuse
  • mental or emotional health changes such as fear or confusion
  • general lack of interest and acting withdrawn
  • changes in personal hygiene or appearance
  • changes in eating and nutritional status
  • behaviour or personality changes
  • poor living conditions, disrepair, clutter and hoarding
