| Description (Service) |
Provides assessment, education, treatment and support for people with diabetes, pre-diabetes, and people that are at risk of developing Type 2 diabetes.
- One-to-One Appointments: Meet with the nurse educator and registered dietitian, who are certified diabetes educators and will provide information about blood sugar testing, medications, activity, nutrition, managing stress and preventing potential problems.
- Group Education Classes: Held at the King Campus, classes include Living Well with Diabetes and Carbohydrates.

Adults at risk of developing or living with diabetes (Type 1 diabetes, Type 2 diabetes, pre-diabetes, and gestational diabetes). |
| Application |
Doctor's referral or self referral required for one-on-one appointments. Contact for information or fill out the online referral form. Bring a list of current medications, blood sugar meter, log book of blood sugar readings, and health card to the appointment. |
