Free community support services coordinated by Ontario Health atHome to promote successful discharges from either in-patient hospital stays or emergency department visits

• provides individuals access to services for a 4-6 week period to regain strength and independence

Services may include:

• frozen meal delivery
• transportation home from hospital
• transportation for 2 follow up medical appointments
• homemaking services
• home safety assessment
• wellness check ins
• communication with other support services
• access to a 24/7 support line
