Disease Prevention Programs

Programs that help people who have a family history or other risk factors associated with a chronic disease such as heart disease and stroke, cancer and diabetes make lifestyle or other changes that can prevent the disease or limit the initial onset. In addition to health promotion activities that encourage healthy living, prevention also embraces early detection efforts, including screening at-risk populations, as well as strong community-clinical linkages to help ensure that people at high risk of chronic diseases have access to community resources and support to prevent, delay or manage chronic conditions once they occur. Some programs may provide preventive care interventions for people who have a broader array of health concerns.

Disability Awareness Training

Programs that offer workshops, discussion groups, activities and other types of training that sensitize participants to people with disabilities or others with visible differences such as scarring that set them apart from their peers, help them experience what it is like to have a disability, and share techniques for positive communication and interaction with people who have disabilities.

Condition Specific Mental Health Information/Education

Programs that provide information through a variety of channels that improves understanding of a particular behavioural issue or mental condition or issue such as schizophrenia or bipolar disorder; the risk factors and signs associated with the condition; screening/diagnostic procedures; treatment and/or management options; the results of the latest research regarding the issue or condition; and general information about strategies that can be employed to support the individual.

Information Services

Programs that provide for the collection, classification, storage, retrieval and dissemination of recorded knowledge for the community. Included are electronic information resources, information and referral programs, information lines, library services, media services, public awareness/education campaigns, research data and rumour control activities.
