Practice focused on fully coordinated, comprehensive and continuity of care through preventive care measures, chronic disease management, mental health counselling, integration with Ontario Health atHome Mississauga Halton, electronic medical record, availability for same day appointments (Advanced Access), coordination and integration with pharmacist Chronic Disease Prevention and Management Program - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions |
