Description (Service) | Reintegration and reconciliation program for individuals and their families through advocacy, education and restorative practices Services include: Discharge Planning: staff meet with clients to assist in developing a post release plan * system navigation and intensive case management services are available to clients for up to two years * includes housing and employment services Drop-In Counselling: staff meet one-to-one with clients * focused on overcoming the obstacles to reintegration * free to individuals with a history of contact with the criminal justice system * recognizes the value of family and friends in the reintegration of ex- offenders Outreach: staff is available to meet clients where they are in the community Peer Mentorship: support clients with their needs and reintegration goals by engaging in recreational outings * mentors help decrease stigma, minimizing isolation and reduce the barriers of oppression |
